The 17 foot crack could have started as a hairline crack between two layers of composite in the booster casing.
Static pressure testing the completed casing before they are filled with fuel might have caught the defect.
Delta 2 rockets often carry space probes, such as Mars Global Surveyor, and the Mars rovers, so I worry when I see one going up in flames like this.
FWIW for all the "why didn't they see the crack" folks, that didn't become obvious until immediately before the motor blew up. Damage during processing and after case test is the official cause.
First off, the minimum safe distance is more than a mile, and nasa clears radius of SEVERAL miles from the launch pad
