Nah, that's just a Boeing Delta II, a simple satellite launcher. No humans were killed but someone's billion-dollar satellite was wasted.

Well the QC on aerospace parts is a bit convoluted and inefficient in the huge amount of paper work involved though they are planning on reforming this.
The 17 foot crack could have started as a hairline crack between two layers of composite in the booster casing.
Static pressure testing the completed casing before they are filled with fuel might have caught the defect.

The people who designed and built the probe that sat atop that rocket must feel like their life's work as all gone. It is difficult to comprehend how upset the team must have been at this.
Delta 2 rockets often carry space probes, such as Mars Global Surveyor, and the Mars rovers, so I worry when I see one going up in flames like this.

actually, it kind of was the fault of the safety inspectors because they missed such a huge detail, but we shouldn't talk crap about them.

I don't know what you'd expect to see other than that. What you're mostly seeing falling down is chunks of solid propellant broken up by the self-destruct system. Six solid motors were firing at the time; 3 weren't, and then there were liquid fuels in the "core" that exploded too.
FWIW for all the "why didn't they see the crack" folks, that didn't become obvious until immediately before the motor blew up. Damage during processing and after case test is the official cause.

First off, the minimum safe distance is more than a mile, and nasa clears radius of SEVERAL miles from the launch pad
